In there there is a test email from me that contains the dataset from the PDF form. Open that form and you should get a popup that asks whether it's OK to link to the original doc sitting on the server. Say yes and the dataset will be superimposed onto the form which if you only have Acrobat Reader you will have to print off.
Let me know if this is all OK please...
